Alternative Level of Care (ALC) in Canada
Patients who no longer require acute care but cannot be discharged to an
appropriate setting immediately are often referred to as “bed blockers”, and
designated ALC. The Ontario
Ministry of Health and Long Term Care (MOHLTC) funding is based partially on
average length of stay (ALOS), which is often quite long for ALC patients.

Common issues in the development of organization wide pathway
system include basic questions such as “what is this and can it fit
comfortably into my practice” to more complex issues in dealing with
expected variability in patient progress, while still tracking and managing
unacceptable variability.

Pathways 101: Evidence-Based Care Management Tools
The following
presentation about basic steps in developing pathways has been presented a
number of times. Following presentation at the Care Management Network a
version is posted on the discussion forum kindly provided by the Grey Bruce

Path Appraisal, an Update
The following
presentation was delivered at the Care Management Network at St Michaels
Hospital Toronto in November 2003 and a version is posted on the discussion
forum kindly provided by the Grey Bruce Network.
The original
Tools for Path Appraisal were developed by J Weinstein, & J Hockley during
1999-2000, presented at a variety of conferences, and used by a number of
organizations. The aim was to support both
beginners and more experienced path teams to identify: where the
organization is in the evolution of using paths; areas of success &
achievement; opportunities for improvement; how to integrate paths with
clinical practice, quality improvement, information management, decision
support and evaluation system in order to use paths to their fullest
potential.
The updated version of the path adds a “Smoke Detector Model” to the
detailed checklist components of the original tool.

Accountability for What? Tracking Patient Activity
This presentation was accepted for the Conference of the Canadian Evaluation
Society, In the Public's Best Interest: Evaluation, Accountability and
Transparency, in Vancouver, June, 2003. Health Care has many hidden
barriers to sound evaluation processes, which pose problems even for those
with many years of healthcare experience. One easy assumption is that
stakeholder group discussions use terms and descriptors with a common
meaning. In a decade of implementation of evidence-based care management
tools, the author has found instead two common problems; terms may have
different meanings, and descriptive phrases may lack precision. Thus,
facilitation processes must include accountability for expressing each
professional’s perception of ordinary terms like ambulation. Such
preparatory work is necessary if data used in quality assurance processes
and other cyclical or focused evaluation activities is to have the internal
consistency to support reliable evaluation processes.
In this presentation, the author will use examples from various settings
illustrating glossaries’ role in supporting an accountable evaluation
process, by defining terms and activities, which support precisely stated
outcome indicators.

Thinking about Evidence-Based Care Management and Decision Making
This presentation has been customized for client presentations at a number
of regional organizations, including the teaching and community hospitals,
and acute care through rehab and community care. Specific organizational
system issues guide the planning of the session. Some organizations wish to
focus on how care management tools can support their utilization management
and decision support processes. Other organizations need to consider
integration with evidence-based decision making or knowledge organization
activities and principles. Here is a handout from one such presentation.
