- do a systematic literature review for evidence about assessment, treatment, and care delivery
- review relevant high-quality tools that have been developed
- identify the involvement of stakeholders (e.g. all key groups such as nurses, patients, physicians, etc.)
- systematically gather the stakeholder view
- review the standards for tools (internally- or externally-developed standards)
- build the draft tool(s) (beginning with the flow of the patients' experience)
- identify key indicators and outcomes
- identify methods for staff to review the draft tool(s)
- incorporate feedback
- develop and enact communication and education plans
- develop a pilot implementation plan
- where appropriate:
- develop a patient and family version of the tool(s) or other patient and family education materials
- seek feedback for refinement from patients and families
- develop education materials for staff, physicians, patients, and students
- review team membership
