- pilot the tool using at least 25-30 patients, if possible (consider representation of units, sites, caregivers)
- identify a project leader or the most responsible person for the pilot
- ensure a process is in place for:
- consistent access to and initiation of tool(s)
- continuous communication, documentation, and variation follow-up
- consistent, timely education and information for patients/families
- documenting potential improvements in the tool(s) and its use
- seek feedback from patients, families, staff, physicians, management, and other stakeholders
- make identified changes, address barriers with input, check for status of indicators against baseline.
