When sharing paths with this site as well as with other organizations, here are some suggested items to include:
1. the goal(s) and objectives for the path,
2. a clear definition or satisfactory description of the patients to whom the path applies,
3. references for the evidence used,
4. an overview of the members of the interdisciplinary development team and a contact person,
5. a list of key indicators and discharge criteria and information or data gathered which you are willing to share,
6. an outline of the process for development,
7. the date of the most recent review and/or set of revisions,
8. an outline of any screening criteria and/or tools used,
9. copies of related tools used, e.g. clinical practice guidelines, routine doctor's orders, medical directives, unit standards, etc.
